Your Medical Records

We would like to advise that The Clinic East Ballina General Practice is currently closed. However, emails are still being monitored and checked by reception staff, periodically. We endeavour to respond to all emails as soon as practicable. We can be contacted on mail@the-clinic.org or by pressing the button below.

Please do not send us emails that require an urgent response. If you are unwell or have any clinical concerns, please contact an alternate medical practitioner. For medical emergencies please always call 000 or attend the Emergency Department.

If you are requesting the transfer of your medical records to another medical practice, please be aware that The Clinic East Ballina cannot release medical records unless we have received a patient-signed medical request form from the requesting medical practice. We are happy to facilitate a secure transfer once formal requests are received. This is for your protection and a legal requirement.

Most medical practices only require a copy of your Medical Care Summary sheet and we are happy to transfer this on a complimentary basis. In exceptional circumstances, where a full copy of your medical notes is required, please be aware that there will be an Administration Fee of $35 + GST. If a full copy is required, please email us and an invoice will be sent out to you. The fee must be received prior to the records being transferred.
